After Aurelio takes John’s Mustang for repairs, John is visited by the Italian crime lord Santino D’Antonio. It is revealed that to complete his “impossible task” – which allowed him to retire and marry Helen – John asked D’Antonio for help. As a form of contract, D’Antonio swore to John a “Promissory,” an unbreakable promise symbolized by a “blood covenant” medallion. D’Antonio presents the medallion to demand John’s services, but John refuses, claiming he is “retired”. In retaliation, D’Antonio destroys John’s house with a grenade launcher.
Winston, the owner of the Continental hotel in New York, reminds John that if he rejects the Promissory, he will be violating one of two unbroken rules of the underworld: do not kill on continental sites and Promissory notes should be honored. John accepts his appointment and meets with D’Antonio, who is responsible for murdering his sister Gianna D’Antonio so he can claim his place at the “High Dome”, a council of high crime lords. D’Antonio sends Ares, his personal bodyguard, to follow and keep an eye on John.
In Rome, John infiltrates Gianna’s party and confronts her. Faced with certain death, Gianna chooses to commit suicide. As he retreats, John is ambushed by D’Antonio’s men, who intend to “tie loose ends” by killing him. After killing most of them, John is chased by Gianna’s bodyguard, Cassian. Their brutal fight is interrupted when they collide in the reception area of the Continental Rome. While the two reluctantly share a drink, Cassian promises revenge for Gianna’s death.
After John returns to New York, D’Antonio opens a $ 7 million contract for John’s death under the pretext of avenging his sister, leading numerous assassins to attack him (unsuccessfully). Cassian confronts John once again on the subway. John wins the ensuing fight, but saves Cassian’s life out of professional respect. Injured and desperate, John seeks help from underground crime lord The Bowery King, whose subordinates treat his wounds and guide him to D’Antonio’s location. John kills the men of D’Antonio, except Ares, whom he leaves immobilized, but D’Antonio is able to escape to the Continental. But instead of discreetly hiding inside one of the luxuriously appointed suites until John Wick finally leaves, D’Antonio allows his unbridled arrogance to become his own destruction. Supremely self-confident about John Wick’s self-restraint within the walls of the Continental Hotel, D’Antonio allows himself to be easily found. He enjoys a meal from the haute cuisine while smugly makes it clear that he intends to stay there indefinitely. John Wick’s anger steals over this and, despite Winston’s warning, impulsively he shoots and kills D’Antonio right there in the dining room.
The next day, Winston goes to John and explains that, according to the High Summit, the reward offered by Wick was doubled and offered globally. John is “excommunicated” and loses all access and privileges to the resources of the underworld. Winston takes time to announce John’s excommunication and his reward position globally for an hour to give him an advantage and provides him with a Promissory Note for future use. Before leaving, Wick pulls his bloody and injured body for a final pronouncement, which appears in the trailers, that “… I will kill them, I will kill them all!” Suspecting that all the killers across New York will lay eyes on him, Wick then flees on foot with his faithful dog next to him.
